2024 PLAYER WAIVER

To help us more effectively raise sponsorship money we may share participant information with our charity beneficiaries and sponsors.

In consideration of the acceptance of this entry in the 2023 Bay Street Hoops basketball tournament (the “Tournament”), I, for myself, my heirs, executors, administrators and assigns, waive any claims to which I may become entitled for any injury or damage, loss or losses of any kind and hereby irrevocably release and discharge Bay Street Sports Corporation (operator of Bay Street Hoops), KidSport, Drive for Dreams Foundation, the Governing Council of the University of Toronto, the organizing committee of Bay Street Hoops, the directors, officers and members of Bay Street Sports Corporation and all other organizers, sponsors, representatives, officials and volunteers, their agents and employees and any other person or organization assisting in this Tournament (collectively referred to as the “Releasees”) from any claims for damages or injury, loss or losses of any kind suffered by me as a result of my participation in or traveling to or from this Tournament or the Tournament banquet. I further state that I am in proper physical condition to participate in this Tournament and am aware of the nature and potential consequences which may ensue from the risks inherent in the game of basketball and participation in the Tournament or Tournament banquet in general and that participation could, in some circumstances, result in physical injury.

I also give my permission for the free use, publication, distribution and display of my name and picture, as many times as you may wish, in broadcast, telecast, print or any other electronic media.

In consideration of Bay Street Hoops accepting this entry, I, agree to indemnify the Releasees from any liability, claims, causes of action, demands, damages, or any other forms of relief which might be made arising directly or indirectly out of or in consequence of the attendance or participation by me in this Tournament or the Tournament banquet, including all legal costs incurred by the Releasees.
